This questionnaire surveys how active your young adult(s) (aged 13-25 years) with autism is / are, and your attitudes about how important exercise is for them. 

This research study is conducted by Dr Alison Bratt, (contact details at foot of questionnaire), at Medway School of Pharmacy, the University of Kent, plus 3 Masters level research students. All questionnaire data is gathered and analysed anonymously and will only be viewed by the listed scientific researchers.

Questions are included which have been modified from the General Physical Activity Questionnaire (GPAC), which is copyright University of Cambridge / University of Manchester. GPAQ was originally developed from the Primary Care Assessment (PCAS) survey with permission of Dr Dana Gelb Safran.

General Data Protection Regulation (GDPR) Information for Health and Care Research: Medway School of Pharmacy at the University of Kent is the sponsor  and funder of this study based in the United Kingdom. We will be using the information you supply by completing this questionnaire in order to undertake this study and will act as the data controller for this study. This means that we are responsible for looking after your information and using it properly. This study has full ethical approval from Medway School of Pharmacy ethics committee (September 2018).

This questionnaire is completely anonymous and no information which you give can be traced to you.
